Patient Information
The Pediatric Healthy Life Center functions by referral only.
If you are interested in the center’s services, please discuss it with your primary care provider. At that time, your physician may send us a referral to ensure that proper communication occurs between the two clinics.
The referral will contain private information about you such as contact numbers and insurance information. Medicaid application forms are available here.
· Application for Alabama Medicaid

Once the referral is received, an appointment will be scheduled with you and your will receive a welcome package detailing the 1st visit.
We ask that all 1st time patients arrive 15 minutes before the scheduled appointment time in order to complete new patient forms and to verify insurance.
The expected time for the 1st visit is about 2 hours.
At the 1st visit:
Please bring the following:
1. Insurance card(s)
2. Picture ID of parent/guardian
3. Name and date of birth of patient
4. Name and date of birth of parent/guardian
5. A current mailing address and phone number where the parent/guardian can be reached
6. List of the patient’s medications and the dosage or bring the medications with you
7. All of the completed forms from the Welcome package
What do I expect?
· You and/or your child will be welcomed by our nurse.
· In order to perform a complete physical examination, we may ask that the patient be placed in a gown. We are committed to the patient’s privacy. If you have any concerns, please indicate them to the nurse and we will accommodate you as best as we can.
· After the nursing assessment, the patient will see the doctor. A complete examination and evaluation is performed. The doctor will then discuss the assessment and the plan with you and/or your child.
· The nurse will then come and provide you with further educational material and instructions about testing if ordered.
· Please do not forget to schedule a follow up as you are being discharged so that we can discuss all results.
· Results of testing can take up to 2 weeks.
What do I expect at the 2nd visit?
· There will be no need to wear a gown.
· The doctor will then review all of the testing and perform a follow up examination. We will formulate specific goals and plans for you and/or your child’s medical condition.
· Testing to measure you or your child’s calorie needs may be done. It is a Resting Energy Expenditure test. It involves no pain, just breathing through your mouth for 5-10 minutes into a small machine to measure the gases in your and/or your child’s breath.
Why follow up further?
· Weight management is very difficult and frustrating. Due to medical complications, multiple issues may happen that are not directly evident at the 1st or 2nd visit. Many studies of commercial weight management centers show that patients unhappily regain weight and may develop complications after about 6 months.
· If complications are identified, follow up is needed to update the plan of care.
